1. What are the symptoms of capsulitis of the second toe?
Capsulitis of the second toe is a condition that develops in the capsule (bundle of structures around a joint) of the second toe. Capsulitis of the second toe is also called Morton’s Neuroma or Morton’s Toe.
It would be extremely helpful to have a short list of the symptoms of capsulitis of the second toe. I’ve had to diagnose this for my wife as I’m not a primary care doctor.
The following are her symptoms.
Generally, the pain starts off as a sharp pain on the second toe when she first gets out of bed in the morning. If the pain is severe, it can wake her up at night. The pain is usually relieved when she puts weight on her foot e.g. walking around or putting her weight on the affected toe to dress, walk or drive. It also seems to be worse when she has been seated for some time e.g. at a concert etc.
Listening to music makes no difference and neither does heat/ice etc.
It gets worse after a hard or long run ( approximately 10k) or if she walks too far in a day e.g. up hills etc or if she has been on her feet all day at work i.e. waitress work. This is why the most likely diagnosis seems to be psorisis or psoriasis of the second toe with capsulitis due to the symptoms being worse when resting for extended periods such as sitting down for too long at work during busy periods or after an outing with extended walking e.g. a concert etc.
Capsulitis of the second toe is a condition of inflammation in the joint capsule of the second toe. This condition leads to pain, weakness and loss of motion of the joint and surrounding tissues. Capsulitis causes inflammation of the joint capsule around the second toe, especially at its base. The condition causes severe pain, swelling, warmth and redness around the toe, leading to stiffness in movements.
2. What are the causes of capsulitis of the second toe?
Capsulitis is an inflammation of the bursa that underlies the joint of the big toe. The five common reasons for capsulitis are:
Over-use of the big toe during sports activities, including running, high-impact aerobics and swimming (if done in too-tight swimming goggles);
Pronation of the foot during walking (inadequate support for the small bones at the base of the foot), which may be caused by wearing inadequate footwear;
Improper fitting shoes, which cause undue tension on the tendons and ligaments leading to pain in the area;
Inflammatory conditions such as rheumatoid arthritis or gout; and
Risk factors due to injury, such as bruising or dislocating the big toe joint.
3. How can I prevent capsulitis of the second toe?
In addition to increasing your intake of vitamin C-rich foods, wearing shoes that provide optimal cushioning for your feet may help prevent capsulitis of the second toe. Opt for flat shoes, such as sandals or flip-flops, whenever possible. If you wear high heels, limit your time wearing them. If you do wear heels regularly, have a podiatrist check your second toe’s alignment and the fit of your shoes.
Capsulitis is the gradual development of calcium deposits in the capsule of a joint. The first case of capsulitis of the second toe was reported by Funck- Helweg and Lequeux in 1960. The condition was originally called “geloma della falange II del piede” in Italian, gelenkarm der mittelfußphalanx and Gelenkarmintrusion.
4. What can I do to relieve the symptoms of capsulitis of the second toe?
Capsulitis is a condition in which the capsule of the joint becomes inflamed, causing pain and stiffness. It is most common in the joints of the first toe and second toe.
3. What is capsulitis?
Capsulitis is the inflammation of the joints in the foot and toes, especially when it affects the second toe. The second toe joint is the one located furthest away from your other toes and this particular joint is most commonly affected by capsulitis.
The joints in your feet are enclosed in soft protective capsules filled with synovial fluid (liquid). The normal movement of the joints in your foot places small forces on these capsules. When you have a condition like capsulitis, the capsule becomes very inflamed and painful, so that even normal movements of your foot become difficult and painful.
5. What are the treatments for capsulitis of the second toe?
Often conservative treatments are recommended. They include:
Proper fitting shoes and ensuring that the metatarsal pad is placed in the correct position in the shoe,
Improvement of biomechanics with appropriate orthotics,
Resting the foot,
Anti-inflammatory steroid injections,
Physical therapy and exercise, and
Orthotic devices like night splints or padding.
Administration of certain medications by injection or orally is another option and can be successful in many patients, but there can be adverse side effects. Rarely is surgery needed for patients with mild to moderate cases.
What are the treatments for capsulitis of the second toe?
Capsulitis is inflammation of the capsule that surrounds the metatarsophalangeal joint. The capsule in this area is very tight, enabling the joint to maintain a solid grip on the bones. Capsulitis causes pain, redness, warmth, and swelling at the joint. Stiffness or immobility of the joint can make walking difficult.
6. What are the signs of capsulitis of the second toe?
Capsulitis of the second toe is a benign inflammation of the joint capsule and the surrounding soft tissues. It can be a painful condition. Capsulitis can occur at any age but, more commonly, it affects older adults (aged 65 years and over). People with a prior history of gout or rheumatoid arthritis are more susceptible. The second toe is most commonly affected but it can also affect the first or fifth toes. Signs and symptoms of capsulitis of the second toe include: swelling of the joint, tenderness of the joint, limited range of motion, pain in the big toe when walking, redness and warmth of the joint and in severe cases, an infection around the affected toe. If you have any of these signs and symptoms around your big toe you may have capsulitis.

A study, led by Zachariah K. Tumin, PhD and published in the Journal of Foot and Ankle Research, examined how people perceive tight shoes. He said: “Foot pain is one of the most common conditions that people seek medical attention for,” and that “foot pain is often a result of shoes that are too tight or poorly fitting”.
Doctors have previously recommended corrective footwear and insoles, but not many people wear them as they are uncomfortable. Dr Tumin’s research showed that most people would wear shoes that were too small instead.
